Remember, processing of your MPN may be delayed if any item in the borrower section or student section is left blank!
Here are some helpful hints to completing the loan form:
Do not complete this form in pencil. Use a ball point pen or typewriter. Incorrect or incomplete information may cause your application to be delayed.
BORROWER SECTION (To be completed by the parent)
- Item 7 - List your state abbreviation and driver's license number in item 7. If you do not have a driver's license, please complete this item with a "N/A" (not applicable).
- Item 9 & 10 - Be sure to include the lender name and address in item 9. If you do
not know the lender code, you may leave item 10 blank.
- Item 11 & 12 - It is important that we be able to reach you during the process of making this loan and during repayment. Enter your employer's name, city, state and telephone number. If you are not employed, enter N/A.
- Item 13 - Complete item 13 entirely, making sure to list two adult references with different United States addresses. Please do not list a business, employer, instructor/coach, or clergyman as a reference. All requested items, including telephone number, must be complete or your application will be delayed. If a reference does not have a telephone enter N/A in the appropriate space.
- Item 21 - Sign and date item 21 in ink after reading the promissory note. When you sign this document, you acknowledge that you have read, understand, and agree to the provisions in the Borrower Request, Certifications and Authorizations section, the Promissory Note and the accompanying statement of Borrower's Rights and Responsibilities and that you agree to repay the loan in full according to the terms and conditions in the Promissory Note.
